“How do I request an IRMAA Adjustment for Medicare?”
If your income has recently changed due to a major life event, you may be paying more for Medicare than you actually should. The good news? You can request an adjustment. Here’s what you need to know about the Medicare Income-Related Monthly Adjustment Amount (IRMAA) and how to file a life-changing event appeal.

What Is IRMAA?
The Income-Related Monthly Adjustment Amount (IRMAA) is an extra charge added to your Medicare Part B and Part D premiums if your income is above certain thresholds. The Social Security Administration (SSA) determines this amount using your tax return from two years ago.
That means if your income has recently dropped, your current premiums may not reflect your actual financial situation.
What Counts as a Life-Changing Event?
The SSA allows you to request a reconsideration of your IRMAA if you’ve experienced a qualifying life-changing event that caused your income to decrease. These include:
- Retirement or reduced work hours
- Loss of income-producing property
- Loss of pension income
- Divorce or annulment
- Death of a spouse
- Marriage (in some cases)
- Employer settlement payments ending
If one of these applies to you, you may be eligible for a lower Medicare premium.

How to Request an IRMAA Adjustment
To request a reduction, you’ll need to file Form SSA-44 (Medicare Income-Related Monthly Adjustment Amount – Life-Changing Event).
Here’s how to do it:
1. Complete Form SSA-44
You’ll provide:
- The type of life-changing event
- The date it occurred
- Your modified adjusted gross income (MAGI) for the current year
- An estimate for the following year
2. Gather Supporting Documentation
Be prepared to submit proof of your life-changing event, such as:
- A retirement letter from your employer
- A death certificate
- Divorce decree
- Documentation showing loss of income
3. Submit Your Request
You can submit your completed form and documentation online here or by printing out the PDF and completing the steps below:
- By mail to your local Social Security office
- In person (if available)
- In some cases, over the phone with SSA guidance
What Happens Next?
Once submitted, the SSA will review your request and determine whether your Medicare premiums should be reduced. If approved, your IRMAA will be adjusted based on your updated income.
Don’t Assume You’re Stuck Paying More
One of the biggest misconceptions about IRMAA is that it’s fixed—but that’s not the case. If your financial situation has changed, you have the right to request a reassessment.
